DAY CAMP SERVICE AGREEMENT

I, the undersigned, have knowingly enrolled my dog in a Day Camp training program with A New Leash Dog Training, LLC. This program includes the transport, training, care, and handling of my dog.

Liability and Representation
I understand that there are inherent risks associated with transporting, training, boarding, socializing, and working with dogs in general. I assume any and all responsibility for my dog(s) during training, boarding, walking and any other work with A New Leash, assuming due diligence and care have been exercised. I will not hold A New Leash, any members, affiliates or property owners responsible for any injuries or misfortunes (to or by me or my dog) while transporting, training, boarding, walking or any other work, should they occur while enrolled in this program, unless due to gross negligence on behalf of their handler. 

I understand that in enrolling my dog(s) into any training or behavior modification program or class with A New Leash, that the trainer has relied upon my representation that my dog(s) are in good health and have received the necessary vaccinations (or equivalent alternatives) as agreed upon by my veterinarian. I will be able to provide proof of these upon request. The background information that I have provided regarding my dog's past and present behavior is as accurate as I am able to provide. I agree to cancel my dog's participation in Day Camp if they become ill or show any symptoms of potentially contagious illness. This includes but is not limited to diarrhea, vomiting, discharge from eyes and/or nose, coughing and/or sneezing, mange, flea or other parasite infestation, etc. There are no penalties for cancelling due to dog illness. 

 
I hereby release and agree to save and hold harmless A New Leash, any members, affiliates, or property owners from any and all liability, claims, suits, actions, loss, injury or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which I or my dog(s) may sustain or which may be caused in any way by my dog(s). 


I acknowledge, that due to the nature of dogs, my dog(s) may incur minor injuries, including scrapes or bruises, while in the care and control of A New Leash that do not require veterinary care. In the event that my dog(s) requires veterinary attention while left in the care and control of A New Leash I hereby authorize A New Leash to transport my dog to the closest available veterinary clinic. A New Leash shall use its best efforts to contact me while transporting my dog(s). I agree to promptly pay all costs incurred for the treatment of my dog(s).

While working with A New Leash, I will adhere to the training plan and instructions as best I can. I agree not to use any training or management tools that have been cautioned by my trainer including shock or e-collars, plastic or metal prong collars, "bark" collars, ultrasonic correction devices, or other devices intended to startle or cause pain to discourage behavior, or to withhold that they are being used without informing my trainer. I understand that such tools are contraindicated in any training or behavior plan that A New Leash will provide, with regard to the official statement published by the American Veterinary Society of Animal Behavior against the use of punishment in behavior modification and training, and the advice of leading dog and animal training professionals around the world. I understand that the use of techniques other than those advised by my trainer can decrease efficacy of the training plan and inhibit progress.

Payment and Scheduling
I agree to provide payment for services when due. I understand that my dog's Day Camp program dates are not confirmed until payment is made in full. 


Day Camp packages must be used within 6 months of being purchased or the cost will not be refundable. Private Sessions that accompany the Day Camp packages must be used within 1 month of your dog completing their Day Camp program or will be forfeited. I understand that it is advised that private sessions should be completed within two weeks of the last day of my dog's Day Camp program for best results. 
There is no penalty for cancelling or rescheduling a private training session so long as the session is completed within the windows described in the above paragraph. 

If you need to cancel a day of Day Camp for your dog, you must e-mail or text your dog's trainer prior to 9am. Due to an insurance limit on the number of dogs that are allowed to be under the control of A New Leash at any one time, additional dogs cannot be added to Day Camp when enrollment has been filled. Rescheduling is based on availability and at your trainer's discretion. Cancellation for full Day Camp programs must be made at least two weeks prior to the start of the program. Failure to cancel within this time frame will result in a cancellation fee of 10% of the program cost.


Access and Transportation
I agree that I have provided access to my home to my dog's trainer whether I am present or not. A New Leash has provided a lockbox for use during my dog's training program OR I have provided A New Leash with my own lockbox, a key, or a unique code to access my home.  

I understand that all employees of A New Leash are covered by insurance and bonding that protect them from being held liable for any damage to my home or missing items unless proven to be intentional acts. The sole purpose of access to my home is for an employee of A New Leash to obtain and return my dog on their scheduled Day Camp days. No employee of A New Leash will be in my home for any other purpose.

I agree that video, audio, photo, and other surveillance recordings of any employee of A New Leash will not be distributed, posted online, or used in any manner except to verify their presence in my home.

I agree to allow my dog to be transported from my home to A New Leash Dog Training, LLC at 7393 Bush Lake Rd., Edina, MN 55439 for the purpose of their training program. My dog may also be transported to parks and other public places at the discretion of their trainer for the sole purpose of training and socialization relevant to their training program.
I understand that my dog will be transported in a crate or secured in the safest way possible for their size and temperament. Proper ventilation and temperature control will be monitored at all times while crated or being transported by their trainer or any employee of A New Leash.

I certify that I have read and understand the rules and regulations set forth herein and that I have read and understand this agreement. I agree to abide by the rules and regulations and accept all the terms, conditions and statements of this agreement and confirm the truthfulness of the contents of this release form completed by me.

Some programs or classes are not appropriate for all dogs. A New Leash reserves the right to remove or refuse a dog from any of the training programs or classes at any time, with regard for the dog's best interest. Alternatives will be provided to the best of the ability of the trainer.

TRAINING SERVICE AGREEMENT

I further understand that due to the way that dogs interact with one another, cuts or scratches can occur to me or my dog, even though the dogs are carefully supervised. I also understand that my participation in any of these activities is not without risks such as tripping, falling, getting knocked down and jumped on, bitten, scratched, etc.

I also understand that in enrolling my dog(s) into any training or behavior modification program or class with A New Leash, that the trainer has relied upon my representation that my dog(s) are in good health and have received the necessary vaccinations (or equivalent alternatives) as agreed upon by my veterinarian. 

I hereby release and agree to save and hold harmless, A New Leash, any members, affiliates, or property owners from any and all liability, claims, suits, actions, loss, injury or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which I or my dog(s) may sustain or which may be caused in any way by my dog(s). I specifically, without limitation, agree to fully indemnify A New Leash, any members, affiliates or property owners for any and all such liability, claims, suits, actions, losses, injury or damage. I understand that I am solely financially responsible for any damage or harm caused by my dog during training, boarding, socializing, or walking.

I take it upon myself to ask questions and use my own common sense. I understand that I do not have to do anything or allow anything to be done that I do not feel comfortable with. 

While working with A New Leash, I will adhere to the training plan and instructions as best I can. I agree to provide payment for services when due. 

Private training packages that are paid for in advance devalue by half after 3 months of being unused, and are void after 6 months from the date of payment. I understand that if I do not use any or all of the training sessions in my package for 3 months, I can only be refunded half of their value. If I do not use any or all of sessions in my training package for 6 months, no refund will be available to me.
Payments for all group classes are non-refundable regardless of whether or not your dog participates. A refund or credit may be given prior to your participation if the trainer determines your dog is not fit for the class you've registered for. 

In the event of a cancellation, there is no penalty if your trainer is notified at least one hour prior to your lesson. Failure to notify your trainer may result in a cancellation fee of 10% of the session cost. If no notification is given and you either do not show up for your lesson or are more than 15 minutes late, you forfeit the entire cost of the lesson and no make up date will be provided without additional cost. This applies to sessions purchased in a prepaid package as well. 

If you are not able to attend a night of class that you have registered for you must email or text your instructor prior to the start time of the class. Follow-ups and homework will still be sent to you via email if you are unable to attend. Failure to notify the instructor of your absence twice within the same program may result in your removal from the class without refund.
